The financial documents of the person responsible for providing financial support to the learner while studying, such as the grandparents or legal guardian, shall be submitted.
What documents are accepted as proof of financial capacity?
The following documents are accepted as proof of financial capacity:
- Certificate of Employment
- Pay slip
- Income Tax Return (BIR Form 2316)
- Employment Contract
- Certificate of Tax Exemption
- Barangay Certificate of Indigency
Is the Annex F (Affidavit of Family's Financial Capacity) required for all applicants?
Annex F is required for learner applicants that fall under categories that require proof of financial capacity. In the ESC, learners that are under Categories B, C, and D are required to submit Annex F. For the SHS Voucher Program, learners under Categories B, C, and E are required to submit Annex F. What happens if the applicant submits incorrect or inconsistent information?
The application may be disapproved if the submitted information is found to be incorrect, inconsistent, or unsupported by the required documents.
How is the learner's category determined if the learner qualifies under more than one category?

Which learning modalities are allowed in the ESC and SHS Voucher Program?
Learners enrolled in face-to-face or blended learning modalities are allowed to participate in the ESC and SHS Voucher Program. Learners enrolled in full online or full modular delivery are not allowed to participate in the ESC and SHS Voucher Program.
